2023–2024 Enrollment

Required documentation

  • Certified copy of birth certificate or other form of proof for student’s age plus an Affidavit of Missing Birth Certificate
  • Custody/guardianship papers (if applicable)
  • Immunization record
  • Proof of residency
    • Valid driver’s license, mortgage papers, rental agreement, utility bill
    • Any document provided must show current physical address of student
  • If transferring from another school, please provide the following additional information
    • Withdrawal form from previous school
    • Unofficial transcript for students in grades 10 through 12

New enrollment

Please complete the enrollment packet. After you have completed the enrollment packet you will need to submit to Mingus Union High School. Once you have submitted your information - we will contact you if we have any questions, otherwise you are ready to go for Mingus 2023–2024.

Continuing Student

All enrollment information has been posted. We look forward to seeing you for the 2023–2024 school year! Go Marauders!

We will be sending a mailer to our families the week of July 18, 2022, with the documents needed to complete the enrollment process. Your mailing will include an information update sheet with the current information that we have on file. Please review the information. 

If all information is correct, you will need to sign and submit. Once you have submitted your information, you are ready to go for Mingus 2023–2024. If the information needs to be updated - please make the changes to the form, sign, and submit to Mingus.

Immunizations

We require proof of all immunizations as mandated in accordance with the state laws of Arizona (exemptions permissible for medical or religious reasons—please call our office for a form). To ensure the safety of all of our students in the event of an outbreak, we will not allow non-vaccinated students to attend school until the outbreak is over.
